Sole Proprietorship

  • Certificate of Assumed Name - Doing Business As (DBA) Papers, if applicable
  • Proof of Federal Tax ID number in the business name
  • Photo ID of all authorized users on the account
  • Last three years of personal tax returns

Corporation

  • Articles of Incorporation
  • Be in good standing with State of Michigan
  • Corporate Bylaws
  • Proof of Federal Tax ID number in the business name
  • Photo ID of all authorized users on the account
  • Last three years of personal and business tax returns

Limited Liability Corporation

  • Articles of Incorporation
  • Be in good standing with State of Michigan
  • Operating Agreement
  • Proof of Federal Tax ID number in the business name
  • Photo ID of all authorized users on the account
  • Last three years of personal and business tax returns

Organization, Associations and Non-Profits

  • Copy of Bylaws or Charter
  • Proof of Federal Tax ID number in the business name
  • Photo ID of all authorized users on the account
  • Last three years of business tax returns
  • Meeting Minutes

Partnership

  • Certificate of Co-partnership - Doing Business As (DBA) Papers, if applicable
  • Proof of Federal Tax ID number in the business name
  • Partnership Agreement
